I have 5 completed keys and 1 key fragment and no hextech boxes. So that means I have gotten 16 key fragments without getting a single box. I get an S almost every game so it can't be because I win a lot and don't get S.
Is my account bugged?
You can only earn boxes on champs you've not earned a box with during the season. You can see who you've earned a box with on your Champ list on your profile. They have a border around them.

you can only get one box per champ per season. you can see which ones you have gotten chests on by going to champion page in profile and you'll see a border around them
