I turned lvl 5 with a champion and i got an s in the same game. It gave a notification i took a token but i actually never received one...

No, you didn't get a token in the same game, the notification said you can now earn level 6 tokens by getting S- or more.

that's a known bug.

you can't earn tokens in the same game you reached lvl5 even if it displays that you have earned one.
