Ranked Queue Drop Issue

Happened once to me so far, in a ranked draft lobby, the enemy team was on bans, when they locked in their ban it switched to myself to ban, the client booted me from the lobby before I even saw the champion select menu and then gave me the dodging ban for not locking in a ban. Despite the fact I got kicked the second the other team had locked in his/her ban, not after the 30/40 second selection time, but before I even saw that.

Also, the 'accept' button has randomly dropped me from queues a couple of times, as if the search has been cancelled.
