Map Glitch and Galio Ult Failure

The map glitch is back for me and several friends. Thus far it's just been annoying when someone says a champ is mid/top/about to poop on you and they're really mid/top/about to poop on you.

Also, today I saw two galio ult glitches where an allied galio ulted an elise who was in human form (not repelled) and she simply walked away without ever having been taunted during the channel. Both times elise was very nearly in melee range, and there were no other mechanics (spell shield, zhonyas etc.) that would have given her immunity or invulnerability.
