Skillshots pass through low champions, not hitting them

I was playing as Zed the other day and my shuriken (not my shadow's)which would have been the final blow against an enemy Karma, passed through her. Same thing happened once when I was playing Ezreal and my Q passed through the enemy Alistar which would have probably killed him. They both had exhaust btw and where supps.
