Sion abilities not working

I believe it was when i hit level 4, i had a dorans blade, boots of speed, and a few health pots and my q stopped working. After I hit 6 i couldn't use my ult either. I was unable to use either of these until around level 11 when i decided to switch them to smartcast to see if that would help, which it did. I'm not sure if this is or is not related to playing sion, but i also randomly saw glimpses of champs that weren't there. For example, i was walking through the jungle and saw the enemy nidalee pounce over a wall and disappear about halfway over it, then i looked at the map and she was defending their midlane tower about a 5th of the map away from where i saw her.
