Swain ultimate suddenly inactive [GAMEPLAY]

While I was playing Swain in Summoners Rift. At two times my ult suddenly stopped being active. The first time my mana was at around 75% and the second time I had a completely full mana bar. It isn't my keyboard because it doesn't happen to any other champions, nor does it happen when typing or other.
