Bug where quin is stuck in bird form

I was playing a ranked game as quin bot lane on 11/4/15 around 9pm. Our Tahm Kench used his ult and i went into bird form and got into his mouth. when we got to the target location i proceeded to target vi and used the bird forms dash. it was then that i noticed i was stuck in bird form. i tried a mess of things to get out of the bug like, reconnecting, leveling up my ult, recreating the incident by going into tahms ult again, but nothing worked. I then went into a created game with friend (after losing my promotional game because of this bug) and recreated this with the exact same results. SO BOTTOM LINE IS: IF YOU TURN BIRD FORM AND THEN GO INTO TAHM'S ULT TO TRAVEL TO THE LOCATION HE DESIGNATES, YOU WILL GET STUCK IN BIRD FORM FOR THE REST OF THE GAME. not sure if it is worth mentioning or not, but i did have the woad scout skin on as well.

This bug also applies when you are eaten by tahm with his w. Was in a game in valor form, became eaten by tahm while ignited, died, and was stuck as valor for the rest of the game. All abilities worked as they should as valor however i did not have the movement buff and the timer on my ult was not counting down. When hovering over the spell the tooltip read along the lines of, "Spell invalid: no valid target found."

I never knew this, but I would also like to say, It's had something similar in the past, and I'm not sure if Riot has Patched, but if ur in Bird form,and timer is running out, then Fiddlesticks fears you, you WILL be stuck in bird form
