forced dodge .Stuck on ACCEPT/DECLINE screen

when in queue , i get the accept/decline screen . i can hear the champion banning and everything . when my turn comes to choose a champ , i cant because im still stuck on the accept screen . This is the 2nd time i dodge a game forcefully and lose LP because a game bug forces me to.
