Ashe Broken?

I was playing Ashe and her stacks weren't stacking. It was about, 15 minutes in I realized her Q also wasn't available. It would be gray even with 3 points into the ability. It would show up available for use for about 1.5 seconds, and when it was used it did a huge amount of damage with one hit. But naturally, not when the opposing team was around. Did they rework her Q or is this a bug? This is the 3rd time I heard her Q did this.

ALSO! Forgot to mention, I was attacking and had the stacks but they disappeared after .5 seconds and then her Q would go away. I was doing whatever I could and it just wouldn't work.
