Varus glitch 11/23/14

So two different glitches happened this game here: http://matchhistory.na.leagueoflegends.com/en/#match-details/NA1/1642045078/40340122?tab=overview The first was very obvious, around the 20 minute mark I lost all ability to cast my Q spell as varus. I have no idea how it happened but after my 2nd death i could no longer cast it. The second glitch could be just be not knowing how items work, but at the very end of the game Warrick killed me while my GA was off cooldown but I did not revive.
