Random death at extremely low health

So as i was playing with my friend I was in a fight but luckily got away with less than 5 health (not sure of the exact amount). As i made it out i just kept walking safely as the opponent turned away and stopped the chase. I was about to recall and then bam, i died for no reason.

  1. no ignite or damage over time was left on me
  2. happened as multiple champs
  3. happened to my friend noobspace as well
  4. summoners rift
  5. only happened when my health got down to about 1
  6. no neutral minions or monsters were attacking me
  7. i did notice that it happened more often when a damage over time was on me before hand but then the tick damage stopped. About 3 seconds later it would seem to do an extra tick of damage out of nowhere.

this happens to me in game im all ining the enemy laner and they get away so i enter into the chat, "wp" and out of no where they just die and i get the kill and then we had a good laugh about it and kept playing

Happened to one of my games too - we were chasing down an enemy Ahri (I believe) who was at really low health. She got away, but then out of nowhere she died (my teammates thought it was because of minions attacking her, but there were none from our side around and she was dashing towards her own tower).
