I had the "championship ward 2014" skin equipped. But after buying the upgraded ward totem (Greater vision ward/Pink ward) my ward had the "sun disk ward" skin. I don't even own that ward skin. Every other ward had my skin (championship)

I tried it in 3 games. 2 games with the championship ward . When i had the championship ward equipped the ward always spawned as a sun disk ward. I tried it again with the ghost ward and the skin was the bat ward (again don't own the bat ward) .
