I recently played a game as Tristana ADC, with Nautilus Sup, against a Miss Fortune ADC with Sona Sup. Miss Fortune managed to stay ahead of me in level the entire game even during the times when I was ahead in CS and Kills. They were 5 CS ahead and a full level ahead, they had no items, masteries, or runes to explain any EXP difference. This was a huge factor in every fight with them and ultimately got them ahead.

Most likely explanation is that they started with a jungle camp (e.g. gromp or krugs) and did it again once or twice during the laning phase. It only counts for 1 CS (or 2 for krugs) and gives a lot more exp.
