Invisible Mundo Cleavers

Hey, I've just recently come across two games where Mundo would throw his cleavers, but you wouldn't be able to see the cleaver (or trophy) in its projectile path until it would finally hit you. I'm not sure how it happens. So far it's been a random occurance.
