Sion Q/R (Smash/Onslaught)

So I am finding an issue with the reworked Sion's abilities. At times during games I will be unable to use Q or R abilities. I have tried many things to try and fix this like seeing if it was the actual keys (so i switched abilities to other keys which didn't work), It seemed in every match i was unable to use the abilities after my first death so i tried execution which didn't work (thinking dying would reset it). Usually I am unable to use Q or R the entire match after first death but time to time the use of the abilities would come back. It is always to same as well, it is always Q and R at the same time. Time to time it will work again but usually unable to use the abilities for the rest of the match.
