I still have not received the icon despite gifting two friends and the event ended on April 11 at 23:59 PST. It is currently April 13, 16:19 PST.

It can take up to 72 hours (maybe a bit longer) for Riot to send out all the gifts. Lot's of usernames to process. Sit tight :)

You'll receive the icon by April 25th. We're still in the process of having them out! :D
