Not respawning when dying after Yorick ult

I was playing a bot game as Yorick jungle earlier and the strangest thing happened after I ulted myself and tried to fight bot and bot. As soon as I ulted myself, for some reason, I died in ONE HIT from full health from a Sivir Q and did not respawn at all, as if my ult didn't trigger. But the cooldown went off and my ghost spawned before I died, so it was definitely on. I've never seen this kind of bug happen where the player fails to respawn after dying like this, and in such a strange way to for the game to act as if my health was at 0 when i was at full o.o

Luckily, it wasn't in a pvp game but it was still really jarring.
