Auto dodging [Client]

There seems to be a problem where the client throws you out of the champion select and treats it as a dodge. I had selected mundo as the champ i was going to pick, and was doing my runes. First guy on other team picks mundo and as soon as its my turn to lock another champ in, the client dodges the game.

i have a similar problem on occasion, where instead of treating it as a dodge it just says im the one who left. then doesn't penalize me. though it happens during random times of the selection process... and ususaly it flashes so fast in the upper right hand corner that i dont even have time to take a screen shot.
