Disappearing Hextech key

I forged a Hextech key with 3 key fragments, I left the loot page for 2 minutes, came back and the key was missing. I did not open a chest, the key just disappeared!! I had 3 chests before I forged and after the key disappeared, I still have 3 chests, so there's no way a chest was opened.
