Jax bug makes whole team disconnect

I was in a middle of a ranked game, playing as Master Yi. And all of a sudden my team just disconnects from the game. Not knowing what was going on, I just surrendered and took the loss. We were playing against a Jax jungle. I hope this gets fixed, and I'm not sure exactly how the bug works. I just want to let you know about this, and hopefully get it fixed.

There have been mulitple reports of this occuring with Jax. Download the replay. Make a highlight focused on Jax for the ~30 seconds leading up to the mass disconnect. If it can be figured out if it's something the Jax player does to trigger the disconnect, it will be very helpful to the devs in fixing the issue.
