My /surrender is broken

Hi i'm just here to say my /ff or /surrender is messed up. Multiple games now out of the absolute clear blue i will be playing, usually this happens during a teamfight, and a surrender vote will pop up instigated by me that i had no partake in doing. I did not type /ff, nor did i type /surrender, how would i be able to if i'm clicking and pressing spells during a teamfight anyway? I'm fairly positive it doesn't have anything to do with keybindings either because i went into a custom a few moments ago and looked through the Hotkeys section to see if there was anything having to do with a Hotkey for surrender and i wasn't able to find anything, i was just wondering if anyone else is having this issue or if riot or someone could please give me some feedback on this or just some help, because my friend's, random people, and myself included are getting very annoyed by the constant random surrender votes popping up. I'm afraid of it opening up sometime in a game and it costing me a very winnable game because my team just decides to go with it.
