No Match Accept Prompt

I was just sitting in que for a ranked match and after a few minutes of not getting a notification while alt+tabbed. I checked and was no longer in que and had a notice saying I have failed too accept a match too many times. And that further failure will result in penalties.

But the client didn't flash in my windows start bar from activity. And upon trying it again and sitting patiently watching the client. I was taken out of que randomly and told it was due to failing to accept the match. I have no picture since there's literally nothing to show....which is kind of the problem.
