[CLIENT] Screen Stuck on 'Accept!' or 'Decline' in Queue

When entering a game my screen is stuck on the 'Accept!' or 'Decline' screen. I can hear the audio in the background for picks and bans/champ selection but I'm unable to pick anything due to the stuck screen. I then lose LP and am slapped with a leave game penalty because of this... So far iv only noticed this in ranked but I would imagine the issue could be repeated in norms as well.

The issue has only happened twice, in the span of about 5 games. Its unclear to me what causes this, I just exit my previous game, go into the next and this may or may not happen...
