Ivern Ult Bug

I was playing a bot game as Ivern. After daisy died to the top tower I was unable to summon her again. It seemed that the game thought she was still alive as my Ult still was in the command form for directing her as seen in the attached image. It stayed like that for the rest of the game.

EDIT: this appears to be reproducible as it just happened to me again. Again, in a bot game, again dead to something other than an enemy champion, again unusable afterwards.
