Two Vel'Koz Bugs- Forever Ult CD and Late/extra damage

Alright the first bug; if a Vel'Koz activates his ult directly before he/she dies the ult will be placed on cool down for the entire game. There is no way to get the ult back and there is no timer on it. I tried switching keys, smart cast, etc.. nothing worked.

Second this is seen with Vel'Koz's "Void Rift"; after the two stacks of his passive are applied the damage comes later, 1 or 2 seconds after the void rift closes. The damage seems to be naturally lagging or possibly a bug of a third wave of damage is being applied after the first two. -Thanks, Slysyl
