[Gameplay] Fiora - death while in q dash causes tp to not work

I was in ranked solo queue playing Fiora vs. Riven. I noticed that teleport was darkened after my death and was not usable until after I was in combat again. The first time was when I died in the middle of my q dash at 3:22 against the Riven's 3rd q. The second time was also in the middle of my q dash at 11:40 against the Riven's aa.

1. Q dash
2. Die during the dash (before doing damage, but while moving).
3. TP is darkened like if it was on cd, but there is no timer.
4. Get into combat (possibly against the Riven) to be able to use TP again.

I have not tried to replicate this issue, but it did happen twice in one match. I'm assuming the problem is with a death during Fiora's q dash but before damage is dealt. Both fights had different sequence of abilities and had different abilities hit and miss. I did get stunned by Riven's w during combat both fights, but that and the death during my q dash seems like the only two parallels.
