Anivia's passive not working
I've encountered this bug 2 times so far. I was baiting opponents into a set trap with my passive up and ready , but instead of transforming into EggAnivia. I died directly.

You know your passive had a cooldown
I looked at your deaths in the match history for this game and found the spots you might be talking about. You have to wait 4 minutes before you get your egg back and in these two spots the time between deaths were about 2-3 minutes. It's not a bug, you just weren't watching your cooldowns.
