Tryndamere and ga revive
tryndamere can ult while the ga revive effect is activating, causing him to use both with no benefit (so if you miss your ult by a tenth of a second, you just lose both)

This is working as intended. GA has a priority value over Tryndamere's ult, so he will go into GA even if his ult is active. There is a similar interaction with Zilean's ult and GA. If you are to receive fatal damage with both a Zilean's ult and GA on you, Zilean's ult will proc first 100% of the time because it has a higher priority than GA
