GAMEPLAY
So i was playing 
support and during one of the team fights, 
Safeguarded (W) me and the animation stayed on me even after death.
Not entirely sure how the bug is produced but here's how i encountered it:
- Play on SummonersRift
- Have an ally on your team
- Have Safeguard (W)
The animation of 's Safeguard should stay on even after death and when u re spawn
