Queue Bug [Believed to be Client Issue]

I was playing ranked and after a player dodged, I attempted to cancel queue (as to not get the same players as before who would intentionally ban my champ), but because a queue had already been found a pop up appeared telling my request had been denied. I hit accepted, but then I was stuck at the home screen for several minutes. I could still hear the bans coming through, however I could not see the queue, and if i tried to join a game my request was denied. After several minutes of attempting to join a new game, my queue finally loaded up visually. However after that the bans seemed to carry over from blue to red side, giving them 6 bans, by repeating every ban we placed, onto their side as well.

I am not sure as I have not attempted to repeat this glitch, but the steps that occurred previous to this are the following:

  1. Joined queue (someone dodged at last pick, replacing me in matchmaking)
  2. Attempt to cancel queue, as a queue is found, causing a pop-up
  3. Queue failed to load visually, but audio was present
  4. Attempted to join new queue, got following pop-up http://i.imgur.com/oSYiGxU.png
  5. Attempted several more times to join a new queue, original queue finally loaded in visually
  6. After the visual load in of queue, ban phase was just ending and the second bug occurred http://i.imgur.com/88oKB9P.png
  7. After every blue side ban, the red side also had the bans on their side, repeating all 3 blue side bans, giving red side a total of 6 bans. http://i.imgur.com/1OZPcTg.png
  8. Queue finished up and game loaded in fine, everything else was normal and I had no further issues with my client in that game, nor the game after. This is the first time since I began league of legends several years ago that I have ever experienced this glitch, and it has not occurred since. This bug appeared on 3/8 around 10:30-11:00 pm If I failed to mention any crucial information, or you need to know anything about what occurred, feel free to contact me.
