(Client) Queue Dodge Blackscreen

Someone dodged in champ select at the last minute and I go this screen, never seen it before in 3 years of playing LoL. I hit play again and it place me in a match for a second and then went to black screen, threatening to give me a penalty timer if I leave.

Addendum: I had a 1 minute penalty timer upon trying to start a new session.
