I feel some times, the fiora ult healing effect dosen't do what it is supposed to do. Fx i used my ult on a vlad. He was really low. I smited him and he died. The healing effect didn't go off.
In another game, I used ult on yasuo. I landed one q on him and he died, but yet again the fiora ult healing effect didn't go off. Rito please fix this bug
You have to attack one of vitals created by your ult. Smite kill didn't destroyed any so healing effect didn't activated. 2nd time is more challenging to pull off but if you activate your ult on somebody then there are areas inbetween vitals. Standing there and trying to aa won't pop them up so the healing effect won't go off. And try to don't use your ult just before you kill somebody. This ult has something like a loading time (it doesn't work instantly, you have to wait half a second or something like that)
