Reward Chests

I've been getting S and S- scores in games and have not received a chest for my scores. It has been on at least 5 occasions, is this happening to anyone else? and do you have suggestions to fix this?

You can only get a chest once per champion per season.

Yes, I have 4 chest available, and spent IP to buy Nautalis just to get an S/chest on him, got an S, didnt get a chest...

I owned Nasus, didnt have a chest on him, got an S, also did not get a chest.
