Hextech Crafting doesn't work correctly(CLIENT)

I've had several instances where a team or I earn an S grade and have had no chest given. When I view my profile it says that I still have 2 available. I have also not received any key fragments for probably 10ish games. Is there a hidden reason or is it just not working correctly?
