What is wrong with my /ff??

I've been having a certain issue for what i would consider to be months now, where i'll be in a game and we'll just be playing ladida whatever and out of nowhere it will say that i instigated a surrender vote when i did not do any such thing. I especially see this prevalent when it happens when i'm in the middle of a fight, and i'm pretty positive it has nothing to do with a hotkey for it because I've skimmed through the hotkeys multiple times now to see nothing in there signifying any sort of hotkey being available for a surrender vote. It's really beginning to annoy me as well as my teammates during 5s games having to say i didn't do that just for them to not believe me or something. Is anyone else having this issue or is there just something wrong with my keyboard or something?? I'm at the end of my rope dealing with this because i also play a lot of 2v5 custom games with myself and a Beginner bot vs a team of Intermediate bots just to practice my mechanics on champions and such. And now multiple times it has forced a surrender vote by me that i had absolutely no partake in ending the game prematurely when i had no intention of doing so. Please look into this and for the love of god FIX IT!
