Summoner Name as a Damage Effect in Death Recap?

Now, I don't have photographic proof of what just happened in a game I played, but I was just wondering if anyone else has seen what I just saw. Idk if it is a bug or something I just never knew about or foul play so I figured I'd put this here and someone can inform me of what's going on.

Playing in an Aram a Vi on the other team was incredibly powerful. Now in Aram that isn't too uncommon so I didn't really question it but looking at the death recap did make me a bit suspicious. Under the Abilities and Effects that did dmg to me the one that killed me each time was one with no picture and when I hovered over it it did various amounts of damage each time from around 20 to around 700. This damage didn't increase as her level did it just seemed to apply randomly each time she killed me. The weirdest thing for me, however, was the name of the effect: her summoner name.

I've never seen that in game before. A champion doing damage and the ability being listed as the summoner name is strange to me and I've never noticed that happening. Now if this is some fairly common thing I'd appreciate it if someone could explain what it is to me. If anyone at all knows what it is I would appreciate the knowledge.

I asked Vi, "What does summoner name mean?" And she responded with some smart remark about it meaning destroying me. Then when I asked about why I was recieving a damage effect with said name she fell completely silent for the rest of the game. After the game ended (with Vi 18/10/18 and victorious) I asked again about the damage and she laughed and left the room. Now I don't know if she was simply trolling me and didn't know herself what the damage was, if she knew what it was but felt like trolling me by not telling me, or if she was hiding what it was because it shouldn't exist.

So the tl,dr of it is a Vi in game applied a dmg effect with her summoner name and no image and I'm extremely curious to find out what it is.

I've left her summoner name out intentionally to avoid "accusing a player of hacking", or calling her out in any way.
