Q: Should activating her Q just before the phantom-hit cause her to apply a phantom-hit to both of the simultaneous attacks, triggering 4 on-hit effects?
A: No! That would be stupid broken. It already triggers 3.
Q: Should activating her Q automatically trigger a phantom-hit on that attack?
A: No, the item's counter is triggered on-attack. The attack for her Q is a multi-attack, but still a single attack.
