Press Play for match making.
do not select champion just hover your mouse over it and it will display full pic of champ.
match making disbanded cause you failed to select champ (happy 6 minute ban)
alt+tab to another app (dont know if that necessary.
come back and this screen will haunt you till next match making champion select.

This has been a bug for a good 2 years now, doubt it will be fixed any time soon.

There is also another annoying bug where if anyone leave the game for any reason, the message pops up saying "Someone failed to select a champion" even if everyone was already locked in...
