Part way through the game my q and r stopped working. It had a significant impact on the game and we ended up losing. I had to quit and log back in only to lose the game. This is incredibly frustrating especially because this bug was supposed to be fixed. Please fix.
I've had this happen to me. I was in aram and died channeling my q or r. super annoying bug.
one workaround i have heard of (but not tried) is to remap your q and r to different keys, push q r and the remapped keys a bunch and then map them back to q and r. then (supposedly) the problem is fixed for that game.
not a great solution but better than having to qq just to be able to use your abilities.
The same thing just happened to me... It seems his passive has something to do with it. When he dies and when he come back the Q and R wont work correctly.
This is a real bug. Multiple people are having it.
We just need a Mod to confirm it.

it happens right after you die.
I had to log out the match and rejoin to fix it.
I made a post about this days ago, and have seen one almost every day. This new boards way of filtering content means the threads get quickly buried and no one sees them.
This was a bug that plagued several champs with charged q's (xerath varus sion) and has existed for a while, would be nice to know if @riot is aware and working on it.
**edit, actually if you search for sion q and r bug you get page after page of small threads about this but none gathered enough votes or attention to stay on top or get noticed. Pretty lame system if you ask me.
The same thing happened to me on multiple games. I played with Annie and my Q wasnt working, then I played aram with LeBlanc and both her Q and E did not work. But for me it doesn't happen after I die, it just happens randomly.
might be related to the buttons you press during your passive zombie form?
It just happend to me, I noticed it happends when you ult and press any button like QWER after sion ults the champion doesn't respond to my Q or R, and just becomes a sitting duck.
I've played 3 games with Sion in Aram I can never use ULT after second time early game R just completely stops working. Super annoying. For now on if I get a scion I have to reroll to avoid. I have every champion first time I experienced bug on this scale with one. This really needs to be fixed.
I have had this happen to be several times during the Poro King event (I don't play sion a ton outside these kinds of events). However, I can confirm it is not keyboard related. The actual power buttons don't work even if you manually click on them. It also carried over to other games and game modes. It eventually started working again on its own, presumably after I logged out and back in.

This just happened to me in game
Can confirm, just happened to me as well, even though I have my abilities from q,w,e,r bound to 1,2,3,4 respectively. I had the issue where my q (1) and r (4) did not work or respond in game, even though I could use them for the in-game chat. Very odd.
I just had the same problem, and I even told my team this. They assumed it was just my keyboard not working, when I can clearly type the letters into chat just fine. Maybe I should've tried those methods as well, I was also in aram when thus happened to me too.
