Black Screen after champion select ruining ranked games

Immediately after picking nasus with a default skin in a ranked game, the screen goes black and never takes me to a loading screen. After trying to reconnect several times, I keep getting the attached message every time. I would love to have my LP back please, since I lost it and got a leave on my account at no fault of my own.
