Guardian Angel bug

I was playing as illaoi in a 5v5 draft pick and bought a G.A. I later died during a team fight and was revived by G.A. but every time after that death it would not revive me. It did not say it was on cool down so I am assuming it is a bug. I have also seen other streamers encounter this issue such as box box.
