I found a bug where if you have sword of the occult on Sion, you lose 3/4 of your stacks on death instead of 1/2. When your passive form dies, it is considered as you die for sword of the occult, so you lose half your stacks on death AND half your stacks when your passive dies. Just wanting to improve the game :) fix when possible
Honestly, after reading this i'm much more concerned about your choice of Occult on Sion then the bug. Needless to say, its definitely a viable complaint.
IIRC 
also has this problem with his ultimate where the person ulted will lose stacks twice. Pretty relevant considering if someone if fed enough to have Mejai's or Sword of the Occult they probably are gonna get Yorick ulted
speaking of 
bugs, his laugh breaks after your first death. he will do the animation, but no sound will come out. I have been meaning to bring this up for a while now, but i kept forgetting.
They still haven't fixed the bug that actually affects him. I still haven't fully figured out what causes it-I think it has to do with dying while charging an ability, but sometimes after dying I discover I can't use certain abilities. Q is the most common culprit, but it can be E, W, or even R...and in at least one case, ALL of my abilities no longer worked. It requires me to restart League to fix.

I am guessing that instead of losing 3/4 of the stacks you are in fact losing half of his stacks twice. While this equates to the same thing, my guess is that he loses half on his death, and another half at the end of his passive.
Incidentally, I believe you can also use up hexdrinker passive during this time. The same is true when Morde ghosts someone and hexdrinker comes up afterwards (which is super-annoying for whoever's hexdrinker shield you just stole, as it's generally a good item vs. Morde).
Sword of the occult will get removed. So why bother.
I just tried it out, had 20 stacks, suicide in towers, only lost 10 stacks. Maybe it only happens once in a while
