Order-of-magnitude Inaccurate Death Report [Client]

I hesitate to tag this as 'Client' since it seems far more likely to be a 'server' issue, but that's not a recommended tag :p. I apologize as I have no idea how to reproduce this bug (unless it's a feature, which seems unlikely).

So, I started with a fairly healthy champion and, as shown in the screenshot, I lost around 2,000 health in 10.24 seconds. That being said, the report only specifies 360 points of damage which, even when you realize that more than three champions did damage to me, seems far too small (though the percentages seem right).

As someone who takes great interest in the damage-numbers when deciding what to build, this is naturally sort of frustrating. That being said, this is the first and only time I've noticed it so off and typically I am more than satisfied with what damage it includes (both for the total dealt and which sources it chooses to include).

I realize that without a way to reproduce this this post isn't exactly helpful, just thought I'd let you guys know :).

More than 90% of my death summaries are bugged. The damages are plain incorrect (sometimes around 2 digit numbers when they clearly aren't, or sometimes off by around 3x. Often with a death with only one assist, the assist enemy appears twice or the killer appears twice. The name of the champion, as well as the summoner name are also often wrong. Sometimes my own summoner name appears as an assist. And there are still minimized icons and tooltips with text incorrectly substituted.
