I was playing a game yesterday , just hit level 18 xerath when all the sudden while farming the wolves, I noticed my Q stopped working, I went to google right away and found no results. I Tried everything, Changed key binds , actually clicked on the icon, i even killed myself ( lost baron buff and eventually lost the game ) and when i respawned I still could not cast Q,
And Xerath joins Vi and Varus in the Q-disable bug pool. Seems common among champs that have a wind-up and then require a second cast to launch Q.
Happens on the rare occasion, sometimes it even opens into game without being able to cast abilities, from what I know try closing client and re-opening it, I know a minute and a half of down a player but better than not being able to cast q for the rest of the game. Hope someone can answer it better than I could.

Press Alt+q, and then try to cast again. Always fixes it for me.
Just had that happen to me in a game. So freaking annoying.
Yeah, from my experience it happens if you get knocked up while channeling Q. Thats happened to me like, 5 times in the past month.
