It's been known for a long time that death recap is pretty outdated. It's quite a neglected feature and has been known for bugging out and displaying incorrect information. I'm guessing that it could have something to do with it being used quickly with Annie's combo... it's just what she does. Maybe she used one of her really bursty spells nearly the same time as ignite and maybe death recap got confused or something.

Can you ignite your blobs? If so, that may have been the case.
I believe it was the damage done to your passive blobs.
What could've happened was the post-"death" damage was attributed to the move that proc'd your passive, instead of spliting them properly.
In other words #justdeathrecapthings.
