Random exit out of que

Sometimes I don't know why I am in a queue, the game freezes for a few seconds and then I am back to the home screen. When I click play it says that I am currently in a queue but I can't go back into the champion select and this is quite annoying for me and also my team. The problem happens relatively randomly and has happened a few times which is annoying because I have been penalized for it.
