Bug locking in Champions in Team Builder

So yeah, not really positive about this issue, but it happened twice today while trying to play Draven. I went into queue with 3 friends both times, and after prepicking Draven, I went about my business figuring out a ban, and banning. When it came to my option to choose my champion I was talking with my friends and ended up trying to lock in with around 4-6 seconds left. The first time, not paying attention to my not being locked in, I got kicked from the queue and sat wondering what happened. The second time the same situation happened, I had already forgot, and by the time I realized and went to try to repick, i already had 0 seconds lefts, lost 10 lp, and had to wait 30 minutes to play. I just want to play this video game. (Not really sure how common this is, it has at least never happened to me). Has this happened to anyone else? If so how frequently.
