I played the last game required to get mastery rank 5 on Ezreal and didn't get a token(even though the system said I would) for the S- This probably is repeatable on any mastery rank acquiry + s games.
It has to be Mastery 5 at the beginning of the game for you to receive the token.
To add on, the notification that popped up was stating that you are now eligible to earn mastery tokens, not that you actually earned one.
You earn tokens for getting S ranks for mastery 5 level games. For that game, were you mastery 4 Ezreal or Mastery 5? If you earned mastery 5 that game, that means the game you played was mastery 4.
EDIT: It telling you that you earned a token is a bug.
