Game crashes in first minute of match [CRASH]

Hey guys, so I've been having this issue for the past couple of weeks now. Every game I join, no matter if it is a classic or an ARAM, I always get disconnected within the first minute of the match. By the time I reconnect, the match is about 1:30, so it affects my jungling rather heavily. I have no idea what is causing it.

Step 1: Join a game Step 2: Wait roughly 30 seconds or so Step 3: Disconnect then reconnect around 1:30 or so
